You can only attain the Ancestral Infant’s Head much later in the game, once you have access to the Eternal City, and its main area Nokron. You can reach Nokron if you are following Blaidd’s and Ranni’s questline and have the Grand Lift of Dectus activated. After defeating Radahn in Redmane Castle, you will open up the path to Nokron. Keep going through the bridge till you reach a four-point crossing. Take the north path towards the Hallow Horn Grounds. Once you reach there, head northwest till you reach a broken bridge. Jump down to the stadium below and climb the wall that’s on the east. Keep going till you reach the ruins at the very end of the path. You can jump on top of it by using Torrent’s double jump ability. Now that you have crossed the bridge, go inside the ruins to find the Ancestral Follower. Defeat it and you will find a chest behind them. Open it to get the Ancestral Infant’s Head.
Once you have the Infant’s Head, you can find it in your inventory and equip it on your weapon slot.
