There are certain things in the game that once you have accepted, you will not be able to go back on it. In the beginning, choosing your character’s class, keepsakes, or anything that stays on you, will be something you will have to live with for the rest of the game. Luckily, some of the keepsakes can be found while exploring the Lands Between or it doesn’t matter. Also, killing certain NPCs early on will lock you out of the option of completing quests for them and further build upon the story. Killing Patches or Fia before their questline is over will not reveal any further options to meet them again, and you won’t get their help later on.
Killing the Fire Giant will give you the option to commit a cardinal sin. If you accept the option from Melina when you meet her at the Forge of the Giants, you will be asked to burn the Erdtree by either sacrificing Melina or yourself. This has major repercussions, such as killing off various NPCs and turning Lyndell into the Ashen Capital. This will also lock you out of various areas that you could have gained access to before.
Finally, choosing one true ending of the game will lock you out from achieving the others. The only way to truly experience the alternate endings is to start a New Game Plus or go back to the save points to change the outcome.
