To unlock the slayer form, you need to complete the game as a Dark Urge character. While playing as a Dark Urge, you have to embrace every kill you make, except one. There will be a point in the game where you murder one of your companions in your camp, after which you will meet someone called Sceleritas Fel. He will reward you with the Deathstalker Mantle, which is needed for the slayer form. After that, visit the Last Light Inn in the Underdark and save Isobel from the ensuing battle. This is one NPC you shouldn’t kill for now, so try to keep her alive at all costs.
If Isobel is kept alive after the battle, you will meet Sceleritas Fel again. He will order you to kill her, and at this point you won’t have any other choice. Save your progress here, then kill Isobel after a long rest. Now the Last Light Inn will be overrun by monsters as the darkness takes over. Defeat all the enemies inside the Inn, then make your way back to camp. Take a long rest, then meet Sceleritas Fel one last time to unlock the Slayer Form.
