When you meet Gortash in Wyrm’s Rock Fortress, he will warn you about the doppelganger in your team. This skepticism comes from the fact that Orin can shapeshift, so there is a chance that someone in your party is not who they seem to be. But luckily, after this interaction, there is no doppelganger at this moment. You can talk to your party to weed out the false member, but you won’t get much out of them.

As you make progress, you will find Orin in Bhaal’s Temple. Talk to her to know more about her shapeshifting powers. She will then transform into someone from your party, just to give you a taste of what she is capable of. Upon further interaction, she will try to strike a deal with you. You can choose to accept, or side with Gortash instead. Whichever option you pick, you won’t find the doppelganger in your camp after all.

Several NPC encounters will confuse you as you keep exploring the city beyond Baldur’s Gate. It is best to take everything you hear with a grain of salt and try not to trust everyone here.
