You can read about the Ancient Lair and gather clues about finding it when you visit the secret basement in Philgrave’s mansion. Make your way to the beach southwest of the mansion to find a hole in the wall. Enter through the hole and explore the basement till you find the Mummy’s Memories book. This will give you clues on how you can find the Ancient Lair.
To find the Ancient Lair, fast travel to the Undercity waypoint in the City Sewers. Go left and keep walking to the end of the passage. You will find the entrance to the Ancient Lair. To get inside, split your party and use the Greater Invisibility spell. Pass through the main entrance, turn right, and break the wall here using any attack or spell. Explore the secret room to find a book. After reading the contents, you can make your way to Mystic Carrion and defeat him.
How to Get Mystic Carrion Heart in Baldur’s Gate 3
Before leaving the Ancient Lair, explore it and shatter any breakable objects to get rare items. After that, make your way back to Thrumbo and interact with him. During the interaction, pick the first dialog option. You can now pick up the heart of the Mystic Carrion. Destroy the jar, since this was your objective all along. After that, make your way to Philgrave’s Mansion for the final fight with the Mystic Carrion.
