Once it is revealed that Astarion is a vampire, you have a choice to make. You can either let Asterion drink your blood or not in Baldur’s Gate 3.

While you rest at your campsite, Astarion creeps up on you. When you confront him, he will explain that he just needs a taste of your blood to feel stronger. He will also ask you to trust him in the process, and you can stop him at anytime.
Let Astarion Drink Your Blood
You can let him drink your blood, and you will get two persuasion checks to make him stop midway. If you let him drain your blood all the way, you will die. Luckily, you can be resurrected if you have the Scroll of Revivify. Once he is done, an ability called Vampire Bite will be unlocked. During battle, Astarion will bite the opponent to drain their health while restoring his own. It will also increase his hit points while the opponent is subjected to more Piercing damage. Also, as you progress in the game, your chances of romancing Astarion will increase.
Deny Astarion
If you think what he does is barbaric, Astarion will be offended and you might lose approval. It can also get harder to romance him later. But he will still unlock the Vampire Bite ability after his hunt and drinking wild animals’ blood.
