The only thing that Brutal difficulty brings to V Rising is making the enemies extremely hard to beat. Any enemy you encounter will have double the health and new moves that deal more damage than before. V Blood Bosses are no exception, as even with the highest gear level you can still falter at bringing them down. They now will attack with three levels instead of two, and their damage output has increased 70% with 25% increase in Max HP.
But defeating bosses has its perks, as there is an increase in loot output by 25%. Also, item durability loss is reduced drastically by 50%. These perks can help traverse through the tough terrains as you explore further into the new region. You would have to utilize all that you have and work on your build and skillset if you wish to advance through the Brutal stage better
