You will notice a small island off the Cerulean Coast, but since Elden Ring has no option to swim, you won’t get to that location just yet. Rather, there is a secret path that you can take to make your way over to the southern island and ultimately reach the Southern Nameless Mausoleum. You need to first head over to Cerulean Coast West site of grace, which can be accessed on the southern section of the map. If you make your way south of Ellac River Downstream, you can spot the Cerulean Coast site of grace between the blue flowers. From this site of grace, head west to locate Cerulean Coast West site of grace near the shoreline. From there, head southeast following the coastline till you find a large dead tree

If you follow the path downwards from there the tree is, you will notice a hole on the ground. Enter the hole and go all the way down, then follow the path to the exit past the giant crab monster. You will soon reach the small southern island. From there, make your way southeast to find the Southern Nameless Mausoleum.
