If you haven’t located Medusa’s hideout yet, you can follow this guide to locate her. But before you can get to her, you need to get an item to help petrify her. For this, you need to find the Daughter Of The Evening shield. This will help return Medusa’s gaze back at her so she gets stunned instead. To get the Daughter of the Evening shield, make your way to Vernworth Castle Vault. There are several chests inside the vault that you have to scavenge through to find the shield. Now, change your vocation to Fighter so you can equip the shield and enter Medusa’s lair

To petrify Medusa, tell your pawns to wait first, then try to lure her away from your team by attacking her. If she does follow you, she will start attacking you with her petrifying gaze. You will know she is about to use this attack when you spot a red spotlight from her eyes. Instead of attacking when this happens, select the Defend option. This will make your Arisen raise their shield, which will start to emit light and ultimately reflect Medusa’s gaze back at her, thus petrifying her. This will take a while, so you have to stand within Medusa’s gaze till you find her turned into stone. The An Eye for an Eye trophy will unlock if done correctly.
