To get the Flower Made From Scrap Iron item, you have to go through the Rarely Affectionate (Part 2) side quest. After completing the second part of the quest for Clara, you can side with anyone to receive the Flower Made from Scrap Iron item. The item will be received in different ways. If you side with Clara, you will get the item immediately, along with the No One is Answering achievement. If you side with Svarog, you have to go to the Tempering Workshop in Rivet Town to get the item.

To find the Tempering Workshop, head over to Rivet Town and visit the westmost block. Check the tables here for an option to investigate. You will get the Flower Made From Scrap Iron item on one of the tables.
