The most common way to remove dirt and blood is to step into any water body. If you find ponds or lakes nearby, just make your dirty character or party member take a dip. This will clean them up and give them a better appearance. Another way you can clean all the dirt is to take short or long rests. After a rest, your character will look fresh and clean. And finally, spells and water flasks will also help you clear up blood and dirt. Transmutation Spells like Create/Destroy Water can be used on your party members or character to clean them, and tossing water flasks at them will work too.
While taking short or long rests, not only will your character and party members be clean, but you can heal certain battle wounds too. You and your party will sustain wounds from battle, and you can heal them and make your character look pristine again. Just head over to your camp or any clearing and take some rest.
