To know whether an item can be combined, all you have to do is hover your mouse over the item in the shop section. You will notice blue lines appear on the item, which means that it can be fused with another item for crafting. To combine items, just drag the blue lined item from the shop and hover it over the items in your backpack. If there is an item that it can combine with, then the line will attach itself to that item. You need to clear the space around said item and place the two items together for successful fusing. If the items are compatible, then the line will turn from blue to yellow.
You can always check your Recipe section in the main menu to know what items can be combined for each class. If you find some items to be blacked out, you need to find them in the store and fuse them in your backpack. Now all that is left is to play a round as usual. Once you complete it and come back to the build menu, you will find your items combined to give you a new weapon or gear.

Can you combine 3 items in one turn? Lets say I place a corrupted crystal in between a holy armor and a heart container, will they both transform next turn?
If you are planning to craft two different items, i.e., the Heart of Darkness and the Corrupted Armor, then you will require separate corrupted crystals. The recipe calls for Holy Armor + Corrupted Crystal and Heart Container + Corrupted Crystal, so you will require 2 Corrupted Crystals to craft both items since they are separate equipment. But if you are going to use three items to craft one weapon like the Fortuna’s Grace (Bow and Arrow + Lucky Clover + Lucky Clover), then this is absolutely doable by combining 3 items in one turn
