To locate the pressure plate puzzle at Fallen Grave, you need to head between the two resonance beacons here. You can find the resonance beacons south of Fallen Grave region, and between them is the puzzle. Once you reach the puzzle area, you will spot the chest in the middle and some breakable structures all around it. Below we will see how to solve the puzzle
First, using your Levitator tool, pull out a rock from the glowing boulder beside the chest. Take the rock to the left side and toss the rock at the other glowing boulder to reveal an explosive
Pick up the explosive and toss it at the thorny branches that are climbing up the cliff to the side. After the explosive has respawned, pick it up and toss it at the thorny branches on the other side of the cliff on the right side of the treasure chest
Take the explosive and start climbing up the cliff. Once you reach the top, look around for a pressure plate and place the explosive on top of it
A little further away from the pressure plate you will find a breakable boulder. Break the boulder and you will get a cube. Pick up the cube and travel further up the cliff to find another pressure plate. Place the cube on the plate
Finally, jump down to the puzzle area and stand on the pressure plate to unlock the chest
