To get the Ancient Meteoric Ore Greatsword, you need to make your way to the Ruined Forge Starfall Past site of grace. You can find this location within the Scadu Altus region. Travel southwest of Moorth Ruins and go along the main path of Fort of Reprimand. Or you can get to Highland Cross, travel north, then once the path splits take the one that goes south. After reaching the end of the path, travel west to find Ruined Forge Starfall Past.

Enter the ruins and go through it while defeating or avoiding the enemies. You will come across a bridge that will lead you to a lever on the left. Use the lever to unlock the door and go to the next area. Explore the area and go through the tunnel that will lead you to a ladder. Go up the ladder and through the next area, then jump down towards the bridge where the second lever is located. 

As soon as the wall goes down, go through it and immediately turn to the right and pull the lever. Then quickly hop on the wall as it ascends. Hop off the wall once you reach the top and head over to the altar to get the Ancient Meteoric Ore Greatsword
