You will want to find Medusa in Dragon’s Dogma 2 mainly to acquire her head which can be equipped and used on multiple enemies to petrify them. But, defeating the boss does not give you her head. There is a certain way you need to proceed to get her head and even get the unspoiled version which can be used to petrify multiple enemies and get achievements. To get to Madusa, you need to progress in the game until you reach Bakbattahl city and take the path to head south-west as shown in the map below.

Once you get to the above location, look for a cave with trees blocking the path. Get into the depths of the cave and you will see Medusa slithering around.

How to Get Preserved Medusa Head in Dragon’s Dogma 2

The fight itself with Medusa is not that hard. Just stay out of the petrification beam and you should be fine. Climb on top of Medusa using thief claws and reach her head. Now, proceed with stabbing her head, but make sure that you use something that is less powerful and does not deal a lot of damage, the Sphinx Ring is great for this. You need to slash her head off to get the preserved head. The goal is to decapitate Medusa before her first health bar goes to zero. If you manage to do that you will get the preserved head.

If not, you will still get the Withered Medusa Head, which can be used only once. You also get a bunch of other loot from her body.
