To find the Skandha of Form in Forest of Wolves – Front Hills shrine. You will encounter this shrine if you travel along Black Wind Mountain. Once you reach the shrine, turn left and follow the path till you reach the bridge. Jump off the bridge and land on the stream below, then keep going left while following the flow of the water. Eventually you will reach a downward slope
You would recognise this area if you were going for the Bullguard fight. Once you reach the slope, look around for the head of a statue. Investigate it to get the Skandha of Form. For now, there is no use for this key item, so just keep it in your inventory to figure out its use later.
