Mephisto is one of the biggest challenges in Diablo 4 Lord of Hatred; phase one and phase two are both brutal with numerous powerful, arena-wide attacks combined with mechanics that can overwhelm players. To survive against Mephisto players need to be constantly moving, using their mobility abilities well, and have good timing for both phases. Below you’ll find a breakdown of Mephisto’s attacks and transitions between phases, along with some of the best strategies to defeat him.
Mephisto Phase 1 Attacks

The first phase of Mephisto features a set of arena-wide abilities that require players to consistently move about. Players will have to deal with several fireballs marked by black circles being summoned on the ground, projectiles that will find you and track you down, and pools of poison that will send waves out from their location around the arena.
The most dangerous ability Mephisto has in phase one of the fight is an arena-wide blast of red that will explode into a spike-filled cloud of corpses. The bodies will explode after a few seconds, dropping fireballs, here mobility skills are mandatory to survive. In addition, it will perform expanding and contracting red lightning attacks which will force you to carefully negotiate between the gaps of where the lightning is.
Once Mephisto gets down around 33% of his max health, he will chain together multiple mechanics with the snakes going across the arena, turning it into a pure survival-based fight.
Mephisto Phase 2 Guide

Phase two involves facing Mephisto in his original form inside Heart of Hatred after defeating him in phase one. The most important feature of phase two is the skill suppression mechanic. To be able to use your abilities against Mephisto again, you must collect orbs marked on the mini-map as your primary focus.
Mephisto gains several new attacks in this phase, including massive spikes, sweeping claw attacks, blast waves that daze you, and red spike snakes that move across the arena. He can also disappear into the blood pool before resurfacing with exploding corpses that release homing fireballs.
When he reaches roughly 66% health, faces will appear throughout the arena and begin attacking him with projectiles which are then redirected toward you in massive waves. Upon reaching 50% health, he will create a maelstrom, pulling you toward the center before a giant face emerges for an instant-kill bite attack.
Final Phase Tips
The sky will become fiery red when Mephisto has approximately one third of his life left in phase two. The arena will be filled with lightning and will begin to combine nearly all of Mephisto’s possible attacks into one massive wave of damage. This is where the fight will become challenging.
In this safe game plan, keep moving every second, do not put all your effort into hitting your enemy, and only hit them when there is a clear opportunity to safely do so. Final stretches are best suited for hit-and-run tactics or ranged attacks against your enemy. Be patient with your health potions, and hold on until you finally kill them and complete the Lord of Hatred expansion.
