The decorative box can be found in the pharmacy on the first floor of Brookhaven Hospital in the Otherworld version. The map that will help you find your way around this place will be called the Brookhaven Hospital Map. However, the hospital’s layout will be different from what you’re used to.
How to Solve the Decorative Box Puzzle
The code to open the decorative box is 2, 3, 1. You should enter the numbers from top to bottom in the order of Q1, Q2, and Q3. The code is based on the answers to the Trick or Treat Elevator Quiz you took while riding the elevator in the Otherworld version of Brookhaven Hospital. The answer to the quiz doesn’t change, no matter how difficult you set the game. So you can use the same code regardless of your chosen difficulty level.

If you open the decorative box, you’ll get 5 Shotgun Shells and 2 Syringes. These items will be very helpful as you explore the game, especially when you encounter stronger enemies later on.
