You can get this trophy quite early in the game. Once you have access to free roam, make your way to Downtown Brooklyn. If you are having a hard time trying to spot it on the map, it is located on the south-east section of the map. You have to make your way to the furthest south-east section once you reach Downtown Brooklyn to find the Big Apple Ballers Stadium. You can quickly spot it as it is right beside the beach and a carnival.
Once you spot the stadium, head inside to start the trophy requirements. To get the Home Run! Trophy, start from the home base and run across the bases in a counterclockwise pattern. You will be notified of the trophy once you complete the objective.
