To get the Worm follower skin, you should first have Berith’s building unlocked. Berith is the new tailor in the DLC who can craft robes for Lamb. You can unlock the Tailor building by heading over to the HQ’s statue and unlocking the southeast slot in the corner. You will also require 16x spider silk and 8x gold bars to build the tailor’s store. Once you have that unlocked, you can visit Berith and get new clothes for your followers.
Now that you have the tailor building unlocked, you can explore the surrounding area and break any and all the objects here. Hidden behind these objects is the contract for the Worm follower, so you have to do a thorough cleanse of the area. During each playthrough, the spawn points are random. Once you find it, you can explore the Follower Form menu to use it on your followers
