If you decide to save the mayor, Bellara will protest, reminding you that he’s responsible for the deaths of nearly all the villagers in D’Meta. After you choose to save him, the mayor will give the party some advice, but it won’t be very useful. Choosing to save the mayor will earn Neve’s approval, which can help if you want to romance her, but Harding and Bellara won’t approve of your decision. Besides that, it doesn’t impact the main story, but it will prevent you from participating in another side quest later on.

On the other hand, if you choose to let the mayor face the Blight, Bellara and Harding will support your decision, while Neve will disagree. She’ll argue that the mayor wasn’t fully in control of his actions and was influenced by the gods. If you let the mayor face the Blight, you’ll also unlock an additional side quest later in the game. He will transform into a powerful boss that you’ll need to defeat as part of the “A Growing Corruption” quest. While this isn’t essential to the main story, it gives you a chance to level up your character and strengthen your relationships with your party members.
Taking both consequences into account, leaving the mayor to his fate is recommend. Additionally, if you are a Grey Warden, you can cut him down and send him to the Wardens, and will play a minor role in some of the Warden quests later in the game.
