In the Hokkaido mission, Agent 47 is tasked with eliminating Erich Soders, who is undergoing a critical surgery to replace his heart. The “(Not) Malpractice” challenge offers a clever, indirect way to complete the assassination by manipulating the chief surgeon into doing the dirty work himself—accidentally. Here’s how to set the plan in motion, step by step.
Gaining Access to Yuki’s Room

To start the plan, you’ll have to get Soders’ kill list first, which is in Yuki Yamazaki’s suite. First, get a bodyguard disguise. One way to do so is to go down to the cable car station on Level 0, outside the main building. The two guards who patrol there—wait until they get apart from each other, then murder one and push the other off the balcony. Once you’ve obtained the bodyguard uniform, you can freely access Yuki’s quarters.
In the suite, locate the bedroom and look for a sheet of paper on a table. That is the kill list showing Soders’ involvement in giving orders for the murder of various people—which includes the chief surgeon’s father. It is worth obtaining this proof so that you can push the surgeon to the limit.
Becoming the Surgeon

Now that you possess the list of kills, you will need to dress up as a surgeon in order to upload it to the hospital’s artificial intelligence, KAI. Go to Level 3, outside of the rehabilitation rooms. Jason Portman will end up there for therapy. Once the session has started and the guard has departed, slip inside and knock out the surgeon who will be performing on Portman. Vanish and aim to conceal the body.
Under this disguise, you can now access the protected areas, including operating room and KAI control center.
Loading the Kill List to KAI
Go to the operating wing in Level 2. Right next to the control room of the robot surgery is a terminal connected to KAI. Download the kill list through this terminal. Once the list is uploaded, the KAI will automatically notify the chief surgeon of the data, which indicates Soders’ role in killing his father.
This report causes the surgeon to despair, and now his actions become unpredictable. The seeds of the fatal “accident” are planted.
Inducing the Drug Relapse

In order to keep the surgeon under impaired judgment while he is operating, you’ll now have to take him to his secret stash of drugs. For this, change to the pilot disguise of a helicopter. You can find the pilot by the helipad in Level 2, outside of the gardens. Wait for him to be alone, knock him out, and take his uniform. A janitor’s room not too far away has a gardener inside, which you can kill or bypass to not get noticed.
With the pilot disguise on, wait near the helipad. The stranded doctor will approach this location, searching for his medication. Follow him to the cache under the helipad. He’ll steal the medication, again changing his actions and making the fateful error in the operating room.
Enabling the “Accident” to Happen
After he’s administered the medicines, the surgeon will return to the operating room on Level 2 to complete the heart transplant. Keep track of him at arm’s length or watch from the observation deck if you’d like to see a movie-style pan. In his current frame of mind and body, he’ll botch the surgery.
Erich Soders will be sent under the knife, the killing officially blamed on the surgeon’s error—getting the (Not) Malpractice challenge done with Agent 47 never actually laying eyes on the target.
