My ADS reticle still exists but my hip-fire crosshair disappears sometimes and never comes back until I reset the game and that’s only if I’m lucky. Adaptive super-sampling is already turned off (never turned it on in the first place). It’s very annoying.

Are you on PC or console? If the issue is not fixed after a restart, reinstall the game on the console.
